Grade 6/7 IBT Music Application Form
Please complete and return to the school with the IBT Registration cheque of $175.00 by April 19, 2013. Please complete the Cosmo Form and send directly to Cosmo with a cheque for $225.00, post-dated to August 1, 2013.
Grade 8 IBT Music Form
Please complete this form and the Cosmo Application. Return both to the school office with payment to Cosmo of $225.00. Cheque can be post-dated to August 1, 2013.
Cosmo Music Instrument Rental Contract 2013-2014
Form must be completed and sent with cheque for $225.00 directly to Cosmo Music. The cheque can be posted-dated to August 1, 2013.

If your child needs health support services to attend school, complete this form in conversation with your child's school or the board's Special Education department.
