STUDENTS WITH ALLERGIES/MEDICAL ISSUES
IF YOUR CHILD HAS AN ALLERGY, PLEASE GO TO THE "FORMS"SECTION ON THE LEFT SIDE OF THE "ABOUT US" PAGE TO PRINT OFF REQUIRED FORMS. PARENTS OF STUDENTS WITH LIFE THREATENING ALLERGIES OR MEDICAL CONDITIONS MUST COMPLETE THE NECESSARY FORMS AND PROVIDE THEM TO THE OFFICE. IF ANY MEDICATION IS BEING ADMINISTERED TO A STUDENT AT SCHOOL, PARENTS MUST HAVE THE REQUIRED FORM COMPLETED BY YOUR CHILD'S PHYSICIAN. NO MEDICATION WILL BE ADMINISTERED BY STAFF WITHOUT COMPLETED FORMS. It is advisable for all paperwork to be completed prior to your child commencing school. Please contact the school if you have any questions or concerns.
ONLINE FORMS NOW AVAILABLE!!
Due to the increasing number of students who tend to "lose"permission forms, we have decided to post our permission forms online. Please note that all students will continue to receive handouts of trip and permission forms from homeroom teachers, however, if a student has "misplaced"a form and the extras have run out in the office, we will be referring students to our website to print their own required forms themselves. Please visit our "forms"section on the left side of the "About Us" page of our website to view and print forms relating to trips and events. Most organized trips and events are curriculum based and it is strongly encouraged that all students attend the trip/event to enhance their knowledge and grades.
