Asthma Plan

​If your child requires medication during school hours to be able to attend school, please complete this form and submit it to the school office.

School Registration Form

​Complete this form to register your teen for a Peel school. You can fill out this form online, then print it. Your changes cannot be saved.

Communication with parents for students 18 years of age or older

​If directed by an adult student (age 18 or older), all communication by school staff regarding school matters will be with that student only.

​Use this form as a checklist for your teen’s school registration.

Allergy alert emergency plan for students with an EpiPen

​if your child has an allergy for which an EpiPen is required, complete this form and give it to the school office. You can fill out this form online, then print it. Your changes cannot be saved.

Extended Absence Form
If a student will be absent for an extended period of time the “Extended Absence Form” must be completed. Teachers whose classes will be missed will, Wherever possible, list topics, units of study, content and/or assignments that will take place during the absence. Arrangements are also made for missed evaluations.

Negative Consent Form
School activities sometimes include such things as: the photographing of students, posting of student work, and publishing student names. If you do not wish to have any information about your child presented in such a manner, please complete the "Negative Consent Form".

40 Hour Community Service Tracking Sheet
As stated in Ontario Secondary Schools, Grades 9 to 12: Program and Diploma Requirements, 1999 (OSS), every student who begins secondary school during or after the 1999–2000 school year must complete a minimum of 40 hours of community involvement activities as part of the requirements for an Ontario Secondary School Diploma (OSSD).
